Documentation Sessions (Supervisor Session Notes)

To access supervisor session notes, you can either schedule a "Documentation" session or choose "Document" from the upcoming sessions list if it has already been scheduled. 

When scheduling a session and selecting "Documentation," supervisors can write session notes without needing to actively conduct a session. This type of session is ideal for supervision sessions or indirect session where actively collecting data isn't required.

On the other hand, choosing "Session" as the session type enables supervisors to actively participate in a 1:1 session, collect data, and also write session notes during the live session.

 

Writing a Supervisor Session Note 

1. Select the desired "Document" session in the upcoming sessions. 

If you do not see your scheduled session, you will need to schedule it first by selecting "Schedule Session" from the client profile.

2. Select "Begin" 
A preview of the session note will appear. To begin editing, select "Begin."

3. Edit "Session Details" 
Select the pencil to edit the session details. Here you will update the time of the session and confirm/ update any other information. The units will automatically calculate after saving with the session times.  *If you do not edit the session times, the session note will track the times the session note is open.

4. Edit "Session Members" 

Select the pencil icon to edit session members. 

Care Team: This list will automatically populate the members of the care team. Select the "-" icon to remove a name.

Staff Present: Select "+ Add member" to add any members from the care team that were present during today's session.

Additional: Select "+ Add member" to add any other participants in a session, like caregivers or siblings.

5. Update the remaining fields of the session note. 
Update "Modifications to Skill Acquisition Goals," "Modifications to to Behavior Reduction Targets," "Follow Ups," "Other Activities," "Narrative," and "Additional Notes." Any fields that remain blank will not appear in the final version of the note.

6. Review and Sign 
Select "Review & Sign" when ready. Here, you can make any final edits and sign your session note. 

Select "Review & Sign"  Sign and Select the Check Icon to Save Select "Finalize"  Caregivers can sign the session note now or can sign at a later time. 

Pro Tip!

Select "Write in Curriculum" to actively write your session note while reviewing programs! This is great for copy and pasting modifications, copy and pasting graphs, and decreasing clicks moving from one tab to another.
