External Care Team

Update the External Care Team on the client profile to document any collaboration that has occurred with other providers. This can also store contact information for providers that are pending collaboration. 

Remember that this information can be seamlessly integrated into your treatment plans. Keeping this documentation updated will contribute to efficiency and effectiveness when it comes to report writing! 

 

Adding a new provider to the external care team: 

1. Select "Add External Care Team Member" on the client profile. 

2.  Update "Provider specialty," "First treatment date,"  and discharge information if applicable. 

3. Update the provider contact information. 

4. Select if a release of information has been obtained and add notes. Tip: Utilize this field to document any collaboration or attempts to collaborate. 

5. Continue to edit throughout intervention by selecting the pencil icon.
